Our staff arranges coordination of care with discharge planners in the hospitals and rehabilitation centers to facilitate the return of residents to their home. Managed Care at Home Care’s staff of health care professional work with your health care provider to provide continuity of care. We work as a team to provide for all your patient care needs. All services are supervised and coordinated by a Registered Nurse or depending on diagnosis a Physical Therapist. Under the direction of your physician, professionals will work closely with you and your family to provide the care and education necessary for your optimum health outcome. By creating a personalized plan of care, we can make sure your recovery is speedy, comfortable and as complete as possible.

Bernarde obtained her Masters Degree in Nursing and has been practicing in the Bay area for over 13 years. Bernarde has worked as the Director of Nursing for San Mateo Hospital and the distinguished Classic Residence by Hyatt serving a diverse population with world – class care. She brings with her an emphasis on caring for the individual with a customized plan of care. It is the fundamental belief of all Managed Care at Home employees that clients simply heal better at home and more successful in maintaining their wellness long-term. As the Administrator of Managed Care at Home, Bernarde will coordinate all care received and visit client personally to develop a comprehensive healing plan.
